26-014 Reunification and Relocation Program
Department Notice
The purpose of this Department Notice is to notify members that the current model of Journey Home discontinued services at close of business on Friday, January 16, 2026. It has been replaced by the Reunification and Relocation Program, administered by Glide Foundation as of January 19, 2026. This program is a merger of Journey Home and Homeward Bound Programs and offers enhanced relocation services to those experiencing homelessness. This merger was required by Mayoral Executive Directive 25-02.
This program is operated by the San Francisco Department of Homelessness and Supportive Housing (HSH). The program will continue to provide transportation tickets to the city of origin, an overnight stay until travel arrangements are made, basic hygiene supplies and a meal stipend during travel.
Program Criteria:
- Adult or Family experiencing homelessness in SF
- Families will need to demonstrate custody of any minor children traveling with them
- Verifiable connection to the destination city
- Not on probation or parole restricting their ability to leave SF
- Willing to voluntarily participate in the program
- Ability to provide informed consent and demonstrate fitness to travel
- Not used travel and relocation in the last 2 years (waivers available)
Referral to Reunification and Relocation Program:
Officers will not need to verify that participants meet program criteria to refer to the program. If they encounter an individual/family who is experiencing homelessness in San Francisco and are expressing a desire to relocate, members should refer to this program. The program employees are responsible for completing necessary assessments.

*The program will be available 24/7 starting on February 16, 2026 when graveyard shifts begin to be incorporated.
A flyer provided by HSH with information on the Reunification and Relocation Program and a contact number that can be offered, as appropriate, during officers' law enforcement or other duties is attached to this department notice. Digital flyers and palm cards that have been translated as required by the Language Access Ordinance are located on their website
sf.gov/reunification.
Members will be required to review and sign off on a training video for this new program in PowerDMS.
