Department Notice
The following Survivor Benefits Checklist and Resource Contact Information is issued to serve as both a reminder and a resource for members of the department to ensure that each employee has current and up to date beneficiary information on record.
Members are further reminded that it is the individual responsibility of each employee to ensure that all pertinent beneficiary information is correctly recorded and on file.
SURVIVOR BENEFITS CHECKLIST
