24-135 Revised SFPD 105 Reportee Follow-Up Form

The SFPD 105 Repartee Follow-up form was last revised in April 2022. Since that time, there have been some changes within the Department. Therefore, the SFPD 105 form has been updated with the following changes:
- SF Safe, Inc. has been changed to Neighborhood Safety Team
- Night Investigations has been changed to Strategic Investigations Unit
- Anonymous Tip Line has been changed to Human Trafficking Anonymous Tip Line
- Homicide/ Anonymous Tip Line has been changed to Homicide/General Crime Anonymous Tip Line
Form SFPD 105 has been revised and translated into Spanish, Chinese, Russian, and Tagalog. Members shall immediately begin use of the revised SFPD 105 form (Rev. 04/24) and recycle outdated forms. A copy of the revised form is attached to this notice and can be located in the SFPD Forms folder and in PowerDMS.
