This Department Notice introduces the SFPD Medical ID Bracelet Program.

Background
The SFPD Medical ID Bracelet Program was created by Chinatown Foot Beat officers in response to memory-impaired individuals repeatedly going missing.

To break this cycle, officers began issuing SFPD medical ID bracelets to individuals once they were safely located. Each bracelet is engraved with the person's name, emergency contact number, and the related SFPD missing person case number.

What sets this program apart from standard medical bracelets is the inclusion of the SFPD case number, which provides officers with an entire police report of information.

Program Expansion
The SFPD Medical ID Bracelet Program is now available department wide. Officers from all district stations can provide SFPD Medical ID Bracelets to individuals who suffer from memory or cognitive impairment and are at risk of going missing.

Medical ID bracelets are normally associated with a missing person case number. Some people may be at risk but have never been reported missing. In these instances, officers can create a simple miscellaneous investigation report and use that case number for the bracelet.

Bracelet Features

  • Hypoallergenic stainless-steel construction
  • Alzheimer's clasp (requires two hands to remove)
  • Engraved with the individual's name and SFPD case number (to protect privacy)
  • Funded by SFPD officers, the San Francisco Asian Police Officers Association, and community organizations

How to Request a Bracelet
All bracelet requests must come from an officer and must be associated with an SFPD case number.

Use the following email format:

Requesting Officer: Name, Star, Station 
Subject's First name: 
Subject's Last name: 
SFPD Case#: 
Subject's Emergency contact: Name and Phone number 
Bracelet Size: Small, Medium, or Large

Bracelets will be custom made which can take approximately 7 to 14 days and sent via interdepartmental mail to the requesting officer. The officer is expected to hand deliver the bracelet to the family or caregiver.

Voluntary Participation 
Participation in the Medical ID Bracelet Program is optional. Officers may utilize the program at their discretion.
