This Department Notice is to inform members how to report damaged protective eyewear kits for the Department's maintenance of asset records and to notify members that the Department no longer has replacements for damaged protective eyewear kits. 

Each station or unit shall use the attached equipment log (SFPD Form 622) for reporting lost or damaged protective eyewear kits. Copies of the damaged eyewear logs will be scanned and emailed to HSU at [Redacted] for grant reporting compliance. Members shall also follow procedures in DN 25-116 Reporting Property Damage Caused by Members.
