(Update to DN 21-150)

Department members no longer need to send completed collision reports or the printed copies of all attachments to the Crime Information Services Unit (CISU).

In many circumstances, members need to send a collision report to Traffic-related units; Traffic Collision Investigation Unit (TCIU), Collision Complaint Review Officer or San Francisco Traffic Offender Program (STOP). Members shall still send the original collision report to the appropriate Traffic unit along with any citation or required DMV forms.

Refer to DN 25-136 regarding Misdemeanor citations related to Traffic and forward via interdepartment mail to "Traffic Admin" at 1995 Evans Avenue.

Original forms should be booked as evidence into the Property Control Division.

Attachments should be uploaded under the initial case number to Evidence.com.

All CHP555 reports signed by the Officer, (standard format, supplementals and counter report format forms) shall be placed in the "for review" folder on the desktop "Collision reports" folder. Officers should maintain a draft copy on their personal desktop in a designated folder.

A supervisor will sign the traffic collision report. Standard reports populate in the "Approved" folder after being signed by a supervisor. The Supervisor will verify the report is placed in the "approved" folder.

Counter reports and supplemental CHP555 reports need to be dragged and dropped into the station appropriate "Approved" folder by the supervisor after signing.

All approved reports will be electronically removed from the "Approved" folder and automatically transferred to Laserfiche and the TCIU permanent folder each morning.

Reports completed in Crime Data Warehouse (CDW) and the "Approved" collision folder, do not need to be printed and sent to CISU. The IT unit has developed a queue so that CISU receives these reports directly to Laserfiche. Hard copies are no longer needed.

Officers assigned to the Airport shall follow Airport Bureau procedures related to the submission of reports. Any questions or clarification regarding this policy should be made to [Redacted] who will provide additional guidance about the directive.
