Identification and Residency Documents

To confirm age and identity:

  • California Driver license or California ID card

To confirm residency in San Francisco, please present two of the following documents:

  • Gas bill
  • Water bill
  • Electricity bill
  • Home Phone bill
  • Garbage bill
  • Cellular phone bill
  • Cable/Internet bill
  • Credit Card and/or Bank Statement (2 consecutive months’ worth of
  • statements)- Mortgage statements are NOT accepted
  • Lease/Rental Agreement or a notarized letter from homeowner stating
  • under penalty of perjury, that CCW applicant resides at San Francisco
  • residential address
  • Voter Notification Card
  • Filing of homeowner’s property tax exemption

Please note that statements or bills MUST be dated within the last 90 days. We DO NOT accept past-due statements or bills. If documents are in spouse’s name, please provide copy of marriage certificate.

Other applicable, relevant documents:

  • DD214 to verify discharge type and/or reason
  • Court documents (if applicable to application)
