The SFPD 68 - Request to Retain Police Star Upon Retirement has been updated with the following changes:

  • New field for obtaining clearance through IAD
  • Removed field for Deputy Chief of the Administration Bureau approval

Members shall immediately begin use of the new form and recycle outdated forms. A copy of the revised form is attached to this Department Notice and can be located in the desktop file "SFPD Forms" and in the PowerDMS folder titled "Forms."
