The SFPD Form 474 Daily Equipment Sign-Out Sheet was last revised in June 2009. Since that time, there have been some changes within the Department. Therefore, the SFPD 474 Form has been updated with the following changes: 

  • Spike strips and vests have been removed.
  • Catch-poles, stop sticks, and portable batteries have been added.

Members shall immediately begin use of the revised SFPD 474 Form (Rev. 5/26) and recycle outdated forms. A copy of the revised form is attached to this notice and can be located in the SFPD Forms folder and in PowerDMS.
